Support Coordination (PIR)

 

We specialise in working with individuals who have complex health needs and are often receiving services from many different service systems or organisations. Partners in Recovery (PIR)provides tailored support for people with complex needs who would otherwise not be able to access the range of health and other services they need to improve their health and wellbeing.

Eligibility
The eligibility criteria varies for our different programs. Please contact the service region specific to you below to discuss whether our programs are suitable for you.

Referrals
Referrals can be made by individuals, carers, families, mental health professionals or other health care providers. Referrals must be discussed with the individual prior to making the referral. We encourage referrals from people from culturally and linguistically diverse backgrounds, Aboriginal and Torres Strait Islander people, youth and refugees. Referral forms can be accessed through the region websites if not already provided below.
