Transition Support Service

The Transition Support Service (TSS) provides post-release and transition support to people with a mental illness who are released from custodial facilities located across South East Queensland from the New South Wales border to Gympie and as far west as Toowoomba. The service receives referrals from and works collaboratively with the Prison Mental Health Service.
