About the program

CPS assists people to live independently, safely and productively in their community, form meaningful connections in a supportive environment, and reduce the need for acute care.

Supports might be delivered individually or in a group setting and aim to support people to achieve their recovery goals.

This service is supported by Central Queensland, Wide Bay, Sunshine Coast PHN.

Eligibility

To be eligible for this service, you need to be:

Aged 16 years and over

Living with a moderate to severe mental illness and have reduced psychosocial functioning

Not receiving assistance through the NDIS or Continuity of Support

Not a participant of a mental health program funded by Queensland Health

Access

This service can be accessed through GP referral, clinical team referral, referral by another service provider or self referral.

Connect

you to relevant local supports and activities and help you to participate in your community
