About the program

HOW DOES IT WORK?

There are two key components to the SAGE Program: 

1. NewAccess 
NewAccess is a free and confidential service for people seeking support to overcome difficult issues and regain confidence.

  • It’s proven, evidence-based and independently evaluated 
  • It provides practical tools to manage everyday challenges 
  • No doctor’s referral is required 
  • It’s available via face to face, phone or video call* 

* Access to face to face appointments is limited in some regions 

The service is delivered by specially trained and supervised Coaches via an evidence-based, ‘guided self-help’ model. The model is a short-term therapy program underpinned by Low Intensity Cognitive Behavioural Therapy. Coaches support their clients to set practical goals and guide them through tools and materials that will help them learn how to identify and change unhelpful patterns of thinking and/ or behaviour. The program is conducted over six sessions and three follow up appointments.

2. Psychosocial Support 
Psychosocial Support is available to SAGE clients to help increase their social connectedness through participation in social and recreational activities.  

DOES SAGE OFFER HOME VISITS?

Yes, we visit people in residential aged care settings. Appointments are offered via face to face, telephone or video delivery depending on the individual needs of the client and the setting in which they live.  

Please note that SAGE is not a crisis service and is suitable for individuals with low risk who are seeking short-term practical support.

Eligibility

To be eligible for this service, you need to be:

An adult
(aged 18 years and over)

A resident in a Residential Aged Care Facility

Living in the Darling Downs / West Moreton region

Not currently engaged in other psychological therapy

This service is funded under the Australian Government’s Primary Health Network (PHN) through the Darling Downs and West Moreton PHN. 

Access

Individuals can access this program directly without the need for a GP referral or a Mental health care plan.
